Who should complete it
The proposed examinee should complete this questionnaire personally using their private access link.
Where someone else is arranging or paying for the examination, the private link should be sent to the proposed examinee so they can answer the health, medication, voluntariness and safeguarding questions directly.
Information boundaries
The person paying for the examination will not automatically receive the examinee's detailed answers.
The client may be informed only of the operational outcome of the review, such as whether the examination can proceed, requires clarification, should be modified or has been declined.
